Getting the accommodation ladder back wasn't done as per the Safety Management System plan.

The Moana Chief was about to leave Port of Auckland on December 10th, 2021. They began taking down the ladder when a crew member got hurt. Their leg was stuck between the top and bottom ladder, causing serious injury. The TAIC reported this.

The crew was taking away the ladder, but it fell off the Port platform. The chains were too loose, so the ladder fell down and started pulling the wire. The ladder moved upwards, and sadly, a crew member got hurt. This area had been marked as dangerous before.

They did not follow the rules for bringing back the accommodation ladder. The Safety Management System guidelines were not followed. Some of the safety measures for taking out and bringing back the ladder were not used.

The accident might not have happened if nearby crew members told the winch operator about the person at risk. This could have stopped the accident.

Deploying and getting accommodation ladders can be risky. Ships' crews have to work over the side of the vessel. Telescopic accommodation ladders have many moving parts, which can also be dangerous. We should follow the instructions of the manufacturer to use the equipment safely.
